Payment in full is required with each appointment. I accept cash, check (preferred), MasterCard, and VISA. Your Wage Works, Flexible Benefits Card, and Health Savings Account (HSA) cards are also welcome!
There is a $50 charge for cancellations with less than 24 hours’ notice. Cancellations due to medical emergencies or weather conditions are, of course, exempt from this charge. PLEASE NOTE THAT WORK IS NOT CONSIDERED AN EMERGENCY! Because I see only one person per hour, I do enforce this policy and ask for your understanding.
Many new and established patients do not know whether or not their insurance policy includes reimbursement for acupuncture treatment. Patients will ask me whether or not I take insurance without even knowing whether or not their policy covers treatment.
It is your responsibility to become aware of your insurance coverage before making your initial appointment. Attached is a form to help you in your discussion with your insurance representative. I am happy to review the form with you.
Currently I do not participate as a Preferred Provider with any insurance plan, nor do I bill insurance companies directly. I will give you an itemized receipt at the end of each visit for you to file. You are responsible for payment in full at the time of your visit.
